for your initial visit to our dental office

At Brookview Dental Clinic, your first appointment begins with a comprehensive exam and consultation explaining treatment options.  Oftentimes, we can perform treatment on the same day as your consultation, saving you time.  However, if you have a complex medical history or treatment plan, a second appointment may be required on another day.

In order to help you as best we can, please provide us with the following information by the time of your consultation:

  • X-rays taken at another office within the last 12 months if applicable
  • List of medications you presently take
  • Medical or dental insurance information

IMPORTANT: Parents or guardians must accompany any patients under the age of 18 at the initial consultation visit.

If you have a medical condition that may be a concern during any surgery, please let our office and dentist know.  Such conditions may include, but are not limited to: diabetes, high blood pressure, artificial heart valves and joints, and bleeding issues.  Also keep us updated on the medications you are taking including those for the heart, aspirin, anticoagulant therapy, and more.  If you require taking medication prior to dental cleanings, please also let us know.

x-rays

If you have had X-rays taken within in the past 12 months by your previous dentist, you can have them forwarded to our office. You can also pick them up and drop them off in person if necessary.  Any additional X-rays required can be taken at our clinic.
